When/where can I refine my Equipment it has a level so if I refine those equipment should I remove those if I exceed that specific level ?

For refining just look at the grade, higher the grade the higher level and more damage it will do. Epic and Unique grades are Purple, Honor and Valor I think are Orange or something, not sure and the crafted ones are blue. Normal weapons should be Yellow.
