So I am just wondering about this description:

"Has a chance of giving 100% damage to all monsters within the area"

 

What constitutes this 100%? is this 100% the total HP of the monsters affected or just 100% your ATK? very confused about this.
Also, it says "while physically attacking", would this mean only when physically melee attacking or doing physical type skills(such as IB, BB, Magnum Break etc) or both?

100% of your ATK. It pretty much just deals one normal hit to everything in the AoE.

 

As for "when physically attacking", you were right in saying that any physical attack, including physicals skills, has a chance of proccing it. There are a few translation inconsistencies across weapons, and it's known that a card that procs the use of a skill doesn't work through physical skills despite saying "when physically attacking" (such as Metaling Card's [Divest]), but in this case it should work through ctrl-clicks AND skills.

 

You'll be happy to know that Death Guidance has one of the most retarded descriptions of any item in the game and tons of people have questioned it, so it's not just you XD

Doesn't death guidance proc vampire gift, in which case it should only work with ctrl click attacks since it autocasts a skill?

I believe all procs talked about in this thread are viable from physical hits (Ctrl/Click), skills, AND reflect (valk manteau, etc).  I know that I recover my SP on RK by mobbing low level mobs with moonlight dagger and Valk manteau.

 

I noticed when mobbing with reflect, my red twin edge (fire) procs alot without doing a single skill.

The twin edge proc isn't a skill so it should work when using other physical attacks as well...

Skill procs can only happen when doing ctrl click attacks. Reflected damage is counted as a normal attack by the player, so such procs happen.

On the other hand, moonlight dagger for example does not proc a skill, so it works with any type of physical attack.

If skills truly proc when using other skills, you'd be seeing people autocast bolts when using bowling bash in mobs, among other things

Oh right derp. That proc is a skill and not an effect.

 

Forget what I said. It autocasts [NPC_VAMPIRE_GIFT] Lv1 which is a skill; Same one that SM uses (but Dracula, hilariously, doesn't). It can't be procced through skills.
