Terrain Bug @ Verta Delta. I also died here.
This thing almost cost me 6700 MP. Good thing I have Blue Spinel, but its used for nonsense bug. Hope I have some compensation about this.

As for that osiris screen, that's what happens when you fall through the floor of a dungeon. I had it happen in CoA one time. I guess I was speeding to fast or something... wind pot + ymir+guardian.... tried to run over a small staircase and fell through the map.
