I had that happen to me a few weeks ago. I had to send in a support ticket so they'd just change the flags on my char to make it have the quests she gives done so I could enter Dimmensional Crack; after i completed that quest, Miss Lady appeared and i could continue the quest line.

Contact customer support for assistance with this. Do not use the report system here on the forums to ask for a GM. They can not help you here on the forums. You must file a ticket to reach them for an issue like this.
