"Cannot init d3d or grf has problem" error means that no graphics mode has been selected (often because none are available).
Run Setup.exe in your RO folder (right click and run as admin if on Vista/7/8 - RO and the config programs should be run as admin, always).
Choose an option in the top two menus (graphics mode, and the resolution you want the game to run at)
Do the same with Classic Setup.
If there are no options in one or both of the top two menus, either you don't have working graphics drivers installed, or you don't have a working copy of DirectX installed.
To see if directx is installed, start->run, type dxdiag and hit enter (or - if there's no run option, click start, type dxdiag in search box, and hit enter) en- and see if it opens a directx diagnostics program, if it doesn't, download directx from microsoft.
If that works, directx is installed, and you need to download and install the latest graphics drivers from the manufacturer of your computer (or graphics card if you're not using the original graphics card or built your own computer).

Run the setup programs (both normal and classic - apparently they can both break things) as admin (you run RO as admin right? If not you probably should, unless you're on XP where everything runs as admin), and make sure something is selected in top two menus. If something is, in both setups, and it still behaves like this, try changing the graphics mode (first menu iirc).

That's what you do to fix a different error (audio init failed), which you get if you have no sound drivers installed yet sound turned on.Ragnarok set up > Turn off your sound
