All Stats + 10, Max.HP + 5%, Max.SP + 5%.
Ignores 50% of normal enemy's physical and magic defense.

Do they all have to be +9 /+10 or can it all be +0 in order to get the bonus all status+10, Max.HP/SP+5%?

Dont need to be refined to provide the +10 stats bonus, they can be +0 and still provide the stats effects, however, check carefully because some other effects are activated only when refined at certain level.
