you cant make it, it comes only from random lucky boxes. So the only way to get it is from said boxes (like the current one out has it) or buy it from people in game :x

All slotted mid headgears are from lucky boxes.
The only one that can be made is Elven Ears [1], which are made from Elven Ears and a Spiritual Auger, which was - big surprise - only available from a lucky box (the slotted elvens are account bound, and these were introduced before they made other slotted mid headgears available, and haven't been in boxes since real tradable slotted mids were added)
step 1: get out your credit card

Lemme share the secret!
In step 3, you tell gravity the 3 numbers (4 if it's AmEx) on the back of the card!
