It might be the case that I have no idea how the GTB card works. But, If the GTB card was equipped while Sera's painkiller was used, would it not nullify damage entirely? How effective is this combination, does it work on MvP's?

GTB changes nothing with painkiller.
GTB + painkiller + DR backpack will let you tank 90% of all mvps with ease.

Painkiller reduces damage, it does not eliminate it.
PK reduces magic too, btw.
