Hmmm, stupid question would be is the bar not completely at the left side when its enabled in game? If not then next I would try re-patching. If that doesn't work go into the settings in your RO folder and see if its enabled there too.
Screenshots would be helpful too or maybe someone can Team Viewer to solve your problem

Firstly, which server are you playing on?
Secondly, does the background music play if you enter the command /bv 127?

This. Also, check sound settings in the setup program, and make sure that if you have more than one audio device, that it's not coming out of your headset or something instead of speakers.
