So I've been Premium since the 26th of June. Never since have I got even one Premium Box. I've checked every sixth hour, five minutes after logging in (and I've been logged in for a few hours afterwards as well). I never get a box in my Item Mall "Purchases" tab. Neither have I ever gotten a message that says "You have a Gift Box to redeem!" (according to this guide).
Am I missing something??

and a popup near your minimap 
indicating that you have a gift box to redeem.

Do you have the Premium Account icon in your buff tray?
If so, you should get a message in your system chat
I do have that icon. The system chat also states my remaining time as Premium.
