Does the effect of Hunter Fly Card stacks with the effect of Rideword Hat Headgear or are they separate and do not stack?

They are separated. Each attack you do will have 3% chance of Hunter Fly activating and X% of Rideword Hat.

off the top of my head, you can easily get 4 separate hp leech at once:

thanatos weap (hp/sp leech)
hf card (hp leech)
rwh/vanagard helm (hp/sp leech)
blood sucker (hp leech)

may be more i'm missing, but this is already a pretty sweet setup.

Well, there is incubus/succubus pet.

The Keris dagger also has a built in 2% chance to leech 10% of damage as HP.
