Speak to the Mad Bunny Merchant in Prontera next to the Silvervine NPC's
 

 
There are 2 options:
 

 
The first option allows you attempt to add a slot to a guard, shield, or buckler for 25 Silvervine. Each attempt to slot is a 5% chance.
 

 
The second option will automatically change any slotted guard, shield, or buckler into a Mad Bunny style shield for 400 Silvervine. This is a 100% chance at the Mad Bunny Style Shield. 
 

 
If you choose the second option, make sure your item is equipped first. Your guard, shield, or buckler also needs to be slotted in order to change it into a Mad Bunny style shield. Please note, the refine level will be kept but any cards equipped to the item will be lost upon the change. This is a Mad Bunny style shield, a cosmetic-only change.

Your first question answer is no. Its cosmetic only.

Second question. Refine stays as it is.

Third question. Cards will be removed after accepting.

1.- Just change the cosmetic form of shield nothing else

2.- Yes, change X shield with slot for a Mad bunny shield with slot, same upgrade.

3.- No, the upgrade remain, but the card dissapear.

Maybe try reading the first post again? It clearly states you can only convert SLOTTED buckler/guard/shield.

I tried to a sloted guard, but I forgot to stack the silvervine, she get my guard[1]+4 and didn't get me anything back...

 

don't do it without stack your silvevine or you loose the shield!!!!

i lost my +9 shield from mad bunny, what happen? i though it will give me mad bunny shield and keep the refine level of my shield, but all it did was make my +9 shield disappear and no mad bunny shield. it also didn't take any of my silvervine huhuhuhu, help what to do T_T

i got 2 separate silvervine right now, i got 10 and 809 silvervine
