Start with the ET map. For "variety", change texture set (if easy to do) to like, the one used in GH or something. Just for variety. Maybe invert the elevation so the player is going down instead of up (again, i don't know if this is simple to do, or a BFD). Even just using an ET map as is would be okay though... Make it one set of 25 levels, not 100, the 100 levels gets tiresome.

Now, start the difficulty off at around what it was for the 80's/90's for ET.

Modified monsters will be needed - multiplying atk and max hp by 5-10 would be a good place to start. Maximize the annoyance factor of skills.

Half a dozen randomly distributed Assassin Cross Eremes (the miniboss) throughout the dungeon. Maybe some others in a similar vein - things to catch people by surprise, and enforce good, cautious play. Oh, and if that was too easy to counter, disable maya purple card on the map. ;-)

Floors designed to be obnoxious:

Stuff that does Area bleeding, everywhere, so everyone's running out of SP. Stuff that does area crit wounds (say level 3 - 60% heal reduction), everywhere. Same deal with area slow cast.

Ranged monsters that cast pneuma when hit with ranged damage. Or quagmire on the attacker. Or both, for a real party.
Lots of maps with mixed elements and races, particularly with things that really hurt of that element. Like, oh, Firebreath from high atk monsters, on the same map as something that did a painful waterball. Everywhere that a map is made up of really painful boss-type monsters, throw in some painful non bosses to screw over people wearing alice (so, map full of scary souped up bosses, plus a few lord knights running around). Sprinkle doomed swords or other stuff that breaks gear throughout - ya know, a few gear breaking monsters mixed in on every map. Have enough gear breakage that full coat or a repair-smith is a must.

Don't be gentle with mvp maps. The MVP-every-five-levels could be upheld. Or it might not be - just to keep people guessing.

Modified MVPs should spam dispel to reduce the impact of mental strength. Wide area dispell would be great if's available :-D

For rewards, start with enough existing "good" weapons/armor for there to be something that would be good for each slot for every class, then boost their power enough for someone to care about. Let the modified/souped up monsters drop, say, the weapons and shields, at fairly low rates, leaving the best items - a good headgear (Slotted feather beret, how bout?), good garment, good armor, good shoes - for the final boss.

maybe something in there could drop a tradable Aumdura Benefit?

The point here is to make something that's really got a challenge to it. Something that's hard enough that teams of top-tier players might not be able to complete for months, just like ET was before. 25 floors of hell.
