Try using Lower Ping or Wtfast to reduce your ping.

Are you in Italy?

The long distance might be the cause of the lag. But try to post a tracert with the server to see which part of the server is lagging. Your tracert seems pretty normal.

Try using Lowerping or WTFast - those really help improve performance when connecting from Europe. 250ms ping is playable, but the lag will be noticable.

Also, as a reality check - when you lag, your mouse moves normally, right?
If the mouse "lags" while you're lagging, that indicates that the lag is on your machine, not a network problem; that could be due to other programs hogging resources, or if it's a laptop, it could be overheating and throttling back the CPU speed to reduce heat (RO is really good at making laptops heat up, and some laptops have overheating problems in general. Hit the vents with an air duster, alternating between the air intake and exhaust until no more dust comes out. Recommend to do this outside. On my old (5 years, and then 3 year old) acer laptops, I had to hit them with an air duster every month, otherwise they'd start overheating while playing RO.)

I have the same issue. I use wtfast to get rid of my "lag". Without that program I have a 1-2 second delay, and with the program the game is a hella lot more responsive to actions. I notice this problem on mmorpgs and udp games, so it is most likely a problem with the routing of my connection. This is experience while playing on Ymir from Texas, US.

This is a tracert.

duckmanneo - yup, sounds like it.

Nowadays, most ISPs are trying only to provide decent download speeds, so people can download movies, music, software and porn - because what do the majority of their customers do? Email, Facebook, Instant messaging, and downloading media. If you're optimizing for that, the ping times don't matter that much, and packet loss doesn't matter that much - as long as you get lots of data to the customer fast, they're happy.

In the case of gaming - for something like RO, the bandwidth of a 28.8 modem is sufficient - it's all about latency, and not losing packets. 1% packet loss is fatal for RO. Hell, 0.1% packet loss is fatal for RO. But that's a metric that isn't being emphasized by consumer ISPs anymore. Comcast routes through AS6453 to get to NTT (gravity's ISP). Well, AS6453's servers (owned by Tata Communications) lose packets at peak times, sometimes even off peak. You don't see it when pirating the latest harry pothead movie, and you certainly don't see it playing gameville on facebook. But you try to play RO, and it's unplayable, you lag to hell. Without lowerping, at least.

And the US broadband market is crap compared to that in the UK and europe - if you aren't happy with comcast, what do you do? You tell them to go to hell and you'll switch to... uh... Well there's Verizon FiO$ if you can afford it (who knows if they're even better - It's not like you can carry your laptop into Verizon's offices and get on their wired (not wireless, because wireless is crap for gaming) network and play RO at 6pm to see how well it works. Most of the country you have 1-3 options only. In Europe, I'm told, most customers have 3-4 choices, prices are lower, and speeds are higher.

Lowerping of course helps get around this situation, by giving you a route that hops off your ISP onto a latency-optimized network as soon as possible. It's about the best you can do short of changing your ISP, and for most people it works miracles.

But, that's some of the background on the lag - I definitely think lag on the internet in general is getting worse, and some aspects are getting less reliable (DNS in particular), despite the increasing importance of the internet. Within the past few years, things have gotten worse in terms of latency, but better in terms of peak download speed.
