I need a little bit of help with the Potion Creation skill. Every time I use it I fail no matter what, and no items are consumed, and I want to know if that is normal or am I using the skill incorrectly.

You do not have the required components to make whatever you are trying to make.

You must have the appropriate guide book (available from npc in alchemist guild - not consumed), a medicine bowl (same npc, one used per cast, even if you don't have ingredients or click cancel) and all of the items required to make the potion you select (consumed, assuming you have all of them).

Ya, usually when it actually fails, all the required items will be gone. I was having the same problem as you. I just couldn't figure out why I kept on failing, but none of the ingredients was missing, so I thought to myself "Well, it's cool, if I fail making potions, at least the ingredients still exist in my inventory". How wrong I was.......

Then it turned out I didn't properly solve the quest for "Call Homunculus", so........
