What is the success rate beyond +10 for lvl 3 weapons?

i think its 18% of Lvl3 and 8% for lvl4 till +14

From my own experience, out of 50 of the ores I would get it to +11 at least 6 times. +12 not at all. out of 100 I got it to +12 once.
