I was having this exact same issue and solved it disabling a program called WizMouse.

• Your DirectX version (look up how to use DxDiag in google): DirectX 12
• Device type (Laptop/PC?): PC + laptop
• Type of mouse used (Wifi/USB?): USB
• Do you use more than one monitor on the device? (If the answer is yes please state how many) No
• Are you using a specific gaming mouse application (Yes/No?): no
• Do you play RO windowed or fullscreen? windowed
• Are you using Steam or Regular game client? both

I have same problem on my desktop and laptop. Where i play windowed and can't click anything mostly happens when switching maps. I then need to click out on my desktop or another window. I hope you can fix this. Seems universal problem for windowed play.
