Is this monster token upgrade a guaranteed 100% refine? or like a safe cert where if it fails you lose nothing but zenny and tokens? or like enriched and mighty hammer where if you fail you lose zenny, tokens and lose an upgrade level on the item?
 

Not 100% chance. Costs only Monster Tokens and Zeny. If it fails, it does not degrade or break.

that's odd. remember back in 2014 when tao was in summoner and spotlight happened to be affecting comodo? and too many cards were dropping? i remember this resulting in mvp cards being taken off spotlight effect. so when you say drops, do you mean normal loot? or did this actually revert back and mvp cards are once again affected by spotlight?

Around September we started to let MVP drops be affected by spotlight changes. The reason MVPS were originally removed from spotlights is due to a weird error with EXP values where it would roll over and leave characters with negative EXP in the tens of millions. Since This issue only was experienced with EXP we added MVPs back to the drops and the drops only portions of the spotlight.
